Novo Research Testimonial Consent & Release

By agreeing to let Novo feature your quote, story, or interview from the recent customer research project (the “Testimonial”), you agree that:

1. Rights & use

Novo Platform Inc. (“Novo”) and its affiliates, successors, and media partners may use:

• your statements about Novo and your business,
• any existing recordings, transcripts, photos, or video clips from the research interviews, and
• any new written, audio, or video content you provide for this purpose (together, the “Testimonial Materials”), including your name, image/likeness, voice, job title, and business name/logo. Novo may edit the Testimonial Materials for length, clarity, or formatting (without changing your meaning) and may use them, in whole or in part, in any media (including Novo’s website, social media, emails, paid advertising, presentations, and events), worldwide, for marketing and promotional purposes, on a perpetual basis.

2. Truthfulness & privacy

Your statements will reflect your honest opinions and experiences with Novo. You agree not to include:

• use your business name in connection with the Testimonial Materials;
• use your business logo in connection with the Testimonial Materials; and
• tag your business on social media (e.g., Instagram, LinkedIn) when sharing the Testimonial Materials.

3. Compensation & disclosures

You understand this consent is separate from, and in addition to, any incentive you received for taking part in the original research study.

• confidential account numbers, passwords, or other sensitive security details; or
• other people’s personal information (for example, employees’ or customers’ private data).
  

4. No further approval or payment

‍You waive any right to inspect or approve the final materials or how often they are used. Other than any incentive already provided for your research participation (if applicable), no further payment or consideration is owed to you now or in the future for Novo’s permitted use of the Testimonial Materials.

5. Authority & age

You confirm that:

• you are at least 18 years old; and
• if you are signing on behalf of a business, you are authorized to grant these rights and to allow Novo to use the business name and logo as selected below.
  

6. Release

To the extent allowed by law, you release Novo and its licensees from any claims, demands, or liabilities arising out of or relating to the permitted uses of the Testimonial Materials described in this consent.

7. Permissions

By proceeding, you agree and consent that Novo may:

• No additional compensation is provided specifically for the Testimonial.
• Where required, Novo may disclose that you previously received an incentive for participating in research (for example, “Customer participated in a compensated research study; opinions are their own; results may vary.”).
